Once the page opens up, print the page using the "Print Selection from the
Menu Bar at the top.
Fill in ALL requested information on the page, including a "Brief
Description" of your problem. Attach this form to the radio or include it
inside the box upon sending in your radio for repair.
Send the radio to the address listed at the bottom of the service form. You
will be contacted, if we require any additional information or if there is a
problem, so please don't forget your Email address.
